Thank you for your interest in The Cancer Foundation (TCF) Financial Assistance Program. TCF is a nonprofit organization designed to decrease the financial burden many people with cancer face during radiation and chemotherapy treatments. 

By clicking the button below you will be taken to TCF’s application for financial assistance. Please note that applications must be submitted to the Foundation by a referring professional (i.e. your doctor, a nurse, social worker, or other health care professional involved with your care). Please read the instructions carefully and fill out the application completely. Be sure to attach copies of the bill you would like the Foundation to consider for payment. Also, be certain to list all current expenses and complete income information.

Patient Application

Am I eligible for financial assistance from The Cancer Foundation? 

Applicants must meet the following qualifications to be considered for the Financial Assistance Program (FAP).

  • Reside in the TCF's 19- county service area which includes: Banks, Barrow, Clarke, Elbert, Franklin, Greene, Habersham, Hart, Jackson, Madison, Morgan, Newton, Oconee, Oglethorpe, Putnam, Stephens, Taliaferro, Walton, and Wilkes counties.
  • Must be at least 18 years old.
  • Have a cancer diagnosis as certified by healthcare provider.
  • Be in active treatment or within a six month period of cancer treatment. Active treatment includes chemotherapy, radiation therapy, hormone therapy, or cancer related surgery. OR Patient declines active treatment and is admitted to hospice services.
  • Have household income less than or equal to 250% of the 2017 Federal Poverty Limits. 
  • Must be able to provide proof of income for each person in household over 18 years. 
  • If you currently have no income, please provide a notarized letter stating the reason for no income. 
  • Have resources totaling less than:
    • $9,000 (single individual)
    • $12,000 (couple)
    • $15,000 (family) 

 What We Fund:

  • Gas, electric, water, propane, and phone payments
  • Rent/mortgage payments
  • Insurance premiums (COBRA included)
  • Health insurance co-payments (does not include deductibles)
  • Nutritional assistance
  • Durable medical equipment 
  • Pharmacy prescription costs
  • Transportation costs (gas cards)
